Chronic Disease Management


Every year we aim to provide an annual review for all patients who have long term conditions. These appointments are longer than routine appointments. This is is a great opportunity for you to understand your health and have one to one time with your GP. 
You will be recalled by one of our receptionists if you have the following conditions:
  • Diabetes
  • Cancer
  • Atrial Fibrilation
  • Chronic kidney disease
  • Chronic heart disease
  • Ischemic heart disease
  • Hypertension
  • COPD
  • Asthma
You will either be allocated with a doctor or a practice nurse - depending on the nature of your problems.
All patients will initially see a Health Care Assistant, this is to ensure that the GP/Practice nurse has all relevant information for your annual review appointment. 
Health Care Assistant appointment involves:
  • Blood test
  • Blood pressure
  • Lifestyle questions
  • Foot checks (for diabetic patients only)
Please make sure you have all of your current medication with you at the time of your GP/Practice nurse appointment as the clinician will make sure you have a medication check.
