Medical Certificates

diaryIf you are absent from work for more than seven days you are required to see a doctor at a routine appointment to obtain a medical certificate. The certificate can be backdated so we do not consider this a sufficient reason for an urgent appointment.

Please do not attend an on call clinic to request a medical certificate.

Absences from work due to illness for a period of less than seven days do not require a doctor’s certificate. You can obtain a self certificate from your employer or the local social security office.


Driving licenses/requests for GP to sign application forms

Please leave forms and photographs with a receptionist. They will be ready for collection one week later. Please note that the doctor must have known you personally for at least 2 years to be able to sign the form. A fee will be charged for this work. A list of fees as recommended by the British Medical Association is displayed at reception.
